Congratulations on completing your reimbursement grant project!
If you have paid out your vendors, closed out your permit (if required), and fulfilled all of the requirements outlined in your award letter, please fill out this “After Action Report” to get your grant!
Note: This form is to report completion of a project approved for a reimbursement grant. Do not use this form if you were already paid in advance through one of our other grant programs with less strict reporting requirements. If you are unsure of which grant you were awarded, refer to the award letter you received.
Another option would be to check if you have not yet received any money. If you have not, then this would be the correct form. If you have, then please use the Show and Tell Report.
