Note: This form is to report completion of a project approved with a grant that was paid in advance. Do not use this form if you were approved for a reimbursement grant which has additional requirements for completion. If you are unsure of which grant you were awarded, refer to the award letter you received. Another option would be to check if money has been deposited into your bank. If so, then this would be the correct form. If not, then you will need to submit for reimbursement using the After Action Report which has additional reporting requirements.
